To ensure a smooth and successful appointment, please review the following requirements before you arrive.
1. Bring a Valid Photo ID
Illinois law requires all signers to be identified through “satisfactory evidence.” Please bring one of the following unexpired government-issued IDs:
- Illinois Driver’s License or State ID Card
- U.S. Passport or Passport Card
- U.S. Military ID
- Foreign Passport (must be stamped by U.S. CIS)
Important: The name on your ID must match the name printed on your document.
2. Bring the Complete Document
- All Pages Included: Even if only one page needs a signature, the Notary must see the document in its entirety.
- No Blank Spaces: Ensure all information fields (except the signature) are filled in. You may cross out or write “N/A” in sections that do not apply to you.
- Do Not Sign Yet: You must sign your document in the presence of the Notary. If the document is already signed, you may be required to sign it again.
3. Arrange for Witnesses (If Required)
Some documents (such as Wills or Powers of Attorney) require additional witnesses.
- Check your document for witness signature lines before your appointment.
- Bring your own witnesses if required; witnesses must also bring a valid photo ID and cannot be named in the document or benefit from it.
4. Notarization Pricing
- Standard Notarization: Base service charges are determined by the complexity and type of document. Note: All notarizations include a standard fee of no more than $5 per signature.
